Sample Web Ticket Submission Form

Dear Customer: This is a sample "end user" form that you could use to "survey" end users about their issues. They would go to this page, fill out the form, and when they click "send" Cynergy automatically sets up a ticket and emails your help desk. The end user can log in and view only their tickets, and see the resolution process.

Note: much of this info could be populated with authorized log in.
